Obviously, a long time ago, this was done by "black highlighters." However, more recently, computer programs have a redaction function.
I remember a story from a few years ago (which I'm not going to make the effort to find) where someone made the error of redacting portions of a document by using the black "highlighter" feature on Microsoft Word. Then he declassified it electronically. It took about 10 minutes for someone to figure out the error and undo the black highlighting, thus revealing information that should have been classified.
